Each application must contain the following: 1) Name and contact information of the applicant for the grant; 2) Name and contact information of the individual coordinating the training; 3) Name of vendor, entity, or individual providing the training; 4) Location of the training; 5) Description of target audience; 6) Purpose of the training, including specific learning objectives; 7) Description of any certification a trainee will receive; 8) Projected number of trainees; 9) Cost of the training; 10) Cost to host the training, including food, lodging, and mileage; and 11) Name and contact information of individual coordinating the costs and reimbursements of expenses eligible under SDCL 34-55-5.
