authorization to provide postsecondary education. The application for certificate of authorization to provide postsecondary education, as prescribed by the Secretary of State, shall contain the following fields: (1) Name of institution; (2) Address of institution; (3) Contact person for institution; (4) Addresses of other locations where institution provides classes; (5) Accrediting information; and (6) Institution classification.
