In addition to the conditions to be met by the student in SDCL 13-49-20.7, each recipient shall submit notarized employment affidavits to the state treasurer at the time employment starts and ends. The employment affidavits must include the following information: (1) The current legal name and residency of the award recipient; (2) The name and address of the employer; (3) Employment status; (4) Date of employment; (5) The signature of the employer or the employer's representative, the signer's title, and the date signed; and (6) The signature of the award recipient. Self-employed recipients must submit notarized affidavits to the state treasurer that include a South Dakota sales tax number and the name and address of the business.
