The VALU loan application form shall include the following: (1) The requested amount of the loan and guaranty; (2) The purpose; (3) Additional documentation, including the following information: (a) A cash flow budget; (b) A signed financial statement completed no more than 30 days prior to application; (c) A repayment plan; (d) Tax forms for the two most recent years, including 1040 and 1040F, if applicable; (4) The lender's signature and the date of signature; (5) The applicant's signature and the date of signature.
