An application for a licensed applicator, licensed operator, and public operator's license must include: (1) The applicant name; (2) The applicant's partnership or organization and its permanent address; (3) The applicant's local base of operation and maintenance; (4) The applicant's telephone number; (5) A statement indicating whether the applicant seeks to be certified as a ground applicator, an aerial applicator, or both; and (6) The date and the applicant's signature.
