20:06:55:23Reinstatement

of coverage. An individual whose coverage or benefits under a health plan ended by reason of reaching a lifetime limit on the dollar value of all benefits for the individual must be provided an opportunity for reinstatement of coverage provided that the individual policy or the group health plan remains in force.
