20:10:10:05Requirements for notice of disconnection.

The notice of disconnection sent to a subscriber shall contain a specific explanation of the following: (1) The particular ground upon which the proposed disconnection is based; (2) The telecommunications company's intention to disconnect service unless the subscriber takes corrective action; (3) The corrective action which the customer must take to avoid disconnection.
