The notice of disconnection sent to the customer shall contain a specific explanation of the following: (1) The particular ground upon which the proposed disconnection is based; (2) The utility's intention to disconnect service unless the customer takes corrective action; (3) The corrective action which the customer must take to avoid disconnection; (4) Notice of the customer's right to appeal and where to appeal.
