An applicant for a license must apply on a form provided by the commission. The application may include questions concerning the following: (1) Personal background information; (2) Financial information; (3) Information on participation in legal or illegal gaming activities in South Dakota or other jurisdictions, including foreign countries; (4) Medical information; and (5) Other information as required.
