for declaratory rulings. Any person may petition the board to issue a declaratory ruling by filing a written request that contains the following information: (1) The name and contact information of the person submitting the petition; (2) The specific administrative rule, statute, or order in question; (3) The facts and circumstances which give rise to the issue to be answered by the board's declaratory ruling; (4) The precise issue to be answered by the board's declaratory ruling; and (5) The signature of the person making the petition.
