20:48:03:13Replacement of license.

The board may issue a duplicate license to a licensee, to replace the original license issued, if the licensee: (1) Submits a written request to the board detailing the cirumstances making replacement of the license necessary; and (2) Pays the applicable fee prescribed by chapter 20:48:06.
