The licensed practical nurse fees are as follows: (1) For a licensed practical nurse license by examination, one hundred dollars plus the current fee for a nationally administered examination; (2) For a licensed practical nurse license by reexamination, one hundred dollars plus the current fee for a nationally administered examination; (3) For a licensed practice nurse license by endorsement, one hundred dollars; (4) For the endorsement of a licensed practical nurse license to another state, territory, or country, twenty-five dollars; (5) For a temporary permit, twenty-five dollars; (6) For the biennial renewal of a license, ninety-five dollars, plus ten dollars to fund the nurses' education assistance scholarship program, pursuant to SDCL 36-9-77, and ten dollars to fund the nursing workforce center, pursuant to SDCL 36-9-95; (7) For the reinstatement of a licensed practical nurse lapsed license, fifty dollars, plus the renewal fee; (8) For changing a name on the records of a license, ten dollars; (9) For issuing a duplicate of the original license, twenty dollars; (10) For placing a licensed practical nurse license on inactive status, ten dollars; and (11) For issuing a limited license to a licensed practical nurse enrolled in a reentry program, twenty-five dollars.
