The clinical nurse specialist fees are as follows: (1) For initial license as a clinical nurse specialist, one hundred dollars; (2) For the biennial renewal of a clinical nurse specialist license, ninety-five dollars; (3) For the endorsement of a clinical nurse specialist license to another state, territory, or country, twenty-five dollars; (4) For a temporary permit as a C.N.S. App., twenty-five dollars; (5) For the reinstatement of a lapsed clinical nurse specialist license, fifty dollars, plus the renewal fee; (6) For changing a name on the records of a license, ten dollars; (7) For issuing a duplicate clinical nurse specialist license, twenty dollars; and (8) For placing a clinical nurse specialist license on inactive status, ten dollars.
