To administer the skills evaluation required by § 20:48:16:08, a board-approved nurse aide training program shall: (1) Use a pool of skills evaluation checklists approved by the board; (2) Use a skills evaluator, who meets the qualifications in § 20:48:16:11, to evaluate an applicant's ability to safely demonstrate the performance of a minimum of four skills selected at random by the board from the pool of skills; (3) Require the skills evaluator to advise the applicant, prior to administering the evaluation, that the completed skills evaluation checklists are to be provided to the board for scoring and, upon successful completion of the evaluation, the applicant is to be placed on the board's certified nurse aide registry; and (4) Submit the completed skills evaluation checklists to the board for scoring. Upon receipt of the four skills evaluation checklists, the board shall score each checklist to determine if the applicant safely demonstrated the critical items on a checklist. If an applicant fails to safely demonstrate a critical item on a skills evaluation checklist, the board must provide notice to the applicant of the area not passed and allow the applicant to be re-evaluated on the skill two times.
