20:48:16:12Certified nurse aide registration -- Application -- Title.

The board shall register an individual seeking initial registration as a certified nurse aide if the individual: (1) Applies on the form provided by the board; (2) Submits an affidavit that the applicant has committed no act of misconduct as set forth in § 20:48:16:04; (3) Provides verification of having completed a board-approved nurse aide training program in accordance with § 20:48:18:01; (4) Passes a board-approved skills evaluation in accordance with §§ 20:48:16:10 to 20:48:16:11; and (5) Provides evidence of passing a board-approved certified nurse aide knowledge exam in accordance with § 20:48:16:09. An applicant who completed training that was not approved by the board shall make a request for an equivalency determination on a form provided by the board. If the applicant does not meet the registration requirements of this section, the board must deny the application. Only an individual registered as a certified nurse aide in this state may use the title "certified nurse aide" and the abbreviation "CNA".
