An applicant for licensure by examination shall provide the following to the board with the application: (1) The application fee of thirty-five dollars; (2) A photo of the applicant; (3) A transcript showing graduation from a college of pharmacy approved by the American Council on Pharmaceutical Education; and (4) A government-issued form of photo identification.
