A change in the location, ownership, or name of a pharmacy, or the closure of business as a pharmacy, must be reported to the board at least ten days prior to the change or closure. The pharmacist-in-charge is responsible for reporting changes to the board. If a pharmacy permanently closes, patients must be notified by the pharmacy owner thirty days prior to closure.
