keeping and reporting requirements. A pharmacist authorized under this chapter to administer immunizations shall maintain in the pharmacy, administered for a minimum of five years: (1) The name, address, and date of birth of each patient who received an immunization; (2) The date of administration and site of the injection; (3) The name, dose, manufacturer's lot number, and expiration date of the vaccine; (4) The name and address of the patient's primary health care provider, as identified by the patient; (5) The name of the person administering the immunization; (6) A record of any consultation or other professional information provided to the patient; and (7) The name and date of the Vaccine Information Sheet provided to the patient. The pharmacy must report all administrations of immunizations to the South Dakota Immunization Information System within 14 days of the immunization. The required records as set forth in this section are open to inspection by the board and must be made available upon the board's request.
