20:59:01:02Application form and fees.

An application for initial social work licensure or a renewal of a social work license must be made on a form provided by the board and accompanied by the fees required pursuant to §§ 20:59:01:10 and 20:59:01:10.01. An application is not complete until the required documentation and fees are received by the board. The board must consider an application withdrawn if it remains incomplete one year after its initial submission.
