20:61:01:05Application for reciprocity.

If the application is for an emergency medical services personnel license through reciprocity, the applicant must submit an active and unrestricted emergency medical services personnel license or certificate of endorsement from another state in addition to the documentation required in §§ 20:61:01:03 and 20:61:01:04.
