20:80:09:06Trainee recognition renewal information required.

For renewal of trainee recognition, the following is required: (1) A completed application; (2) The required renewal fee; (3) Documentation of accumulated work hours under board approved supervision completed after the board granted trainee recognition; (4) Documentation of accumulated supervision hours with a board approved supervisor completed after the board granted trainee recognition; and (5) Documentation of accumulated education completed after the board granted trainee recognition. The information required in this section must be submitted to the board annually to renew trainee recognition.
