20:80:12:09Application to reinstate an expired plan of supervision.

An expired plan of supervision may be reinstated within thirty days after the date of expiration. An application to reinstate the plan of supervision must be submitted to the board within thirty days after expiration. No work experience or supervision may be accumulated between the date of expiration and the date of reinstatement of the plan of supervision.
