The board shall collect the following fees: (1) Application fee, fifty dollars; (2) Examination fee, one hundred fifty dollars; (3) Addiction counselor trainee recognition fee, one hundred dollars; (4) Addiction counselor trainee renewal fee, one hundred dollars; (5) Addiction counselor supervisee recognition fee, one hundred dollars; (6) Addiction counselor supervisee renewal fee, one hundred dollars; (7) Addiction counselor certification fee, one hundred dollars; (8) Addiction counselor certification renewal fee, three hundred fifty dollars; (9) Addiction counselor licensure fee, one hundred dollars; (10) Addiction counselor licensure renewal fee, four hundred dollars; (11) Prevention specialist trainee recognition fee, one hundred dollars; (12) Prevention specialist trainee renewal fee, one hundred dollars; (13) Prevention specialist supervisee recognition fee, one hundred dollars; (14) Prevention specialist supervisee renewal fee, one hundred dollars; (15) Prevention specialist certification fee, one hundred dollars; (16) Prevention specialist certification renewal fee, three hundred fifty dollars; (17) Peer support trainee recognition fee; fifty dollars; (18) Peer support trainee renewal fee; fifty dollars; (19) Peer support supervisee recognition fee; fifty dollars; (20) Peer support supervisee renewal fee, fifty dollars; (21) Peer support certification fee; fifty dollars; (22) Peer support certification renewal fee; fifty dollars; (23) Certification reinstatement fee, one hundred fifty dollars; (24) Licensure reinstatement fee, one hundred fifty dollars; (25) Status upgrade fee, one hundred dollars; (26) Dual credential renewal fee, four hundred dollars; (27) International certificate fee, twenty dollars; (28) Inactive certification fee, fifty dollars; (29) Inactive license fee, fifty dollars; or (30) Temporary license fee, fifty dollars.
