required. Any person seeking licensure shall submit a notarized application and the following information: (1) Name, address, social security number, telephone number, and date of birth of the applicant; (2) Documentation of certification as required in SDCL subdivision 36-38-7(2); (3) Official transcripts showing compliance with educational requirements required in SDCL 36-38-7. Applicants are responsible for having an official college transcript sent from the college to the board; (4) A statement of any criminal record; (5) A statement regarding any disciplinary proceedings or pending complaints as required in SDCL 36-38-7; and (6) Any applicable fees.
