The application for a captive game bird license must include: (1) The name and address of licensee; (2) The number of game birds by species and sex to be held in captivity; (3) The legal origin of game birds, including the date of acquisition and the name and address of supplier; (4) The legal description of premises where the game birds will be held in captivity; and (5) The most recent captive game bird license number and year of issuance, if applicable.
