documentation. A wildlife rehabilitator may renew a wildlife rehabilitator permit upon submission to the department of a permit renewal application together with a report of all wildlife rehabilitated, released, and euthanized during the previous calendar year on forms furnished by the department. A permit holder shall maintain a log of each animal taken into custody to include date the animal was received, county of origin, treatment, condition, and disposition.
