Each initial or renewal application must include either: (1) A certification, on a form supplied by the department, that the applicant is not required to obtain any city or county registration, license, or permit; or (2) Copies of all required registrations, licenses, or permits. The department may contact the city or county to verify the absence of registration, licensing, or permitting requirements or to verify the form and content of such documents.
