At the time of the discharge, the ICF/IID shall develop a final summary of the person's developmental, behavioral, social, health, and nutritional status and, with the informed consent of the person supported, a parent if the person is under 18 years of age, or a legal guardian, if any, shall provide a copy to authorized persons and agencies. The ICF/IID shall provide a post-discharge plan of care that will assist the person to adjust to the new living environment.
