48:02:09:01Transportation of video lottery machines into

the state. A manufacturer or distributor shipping a video lottery machine into the state shall provide the lottery in writing at time of shipment the following information on forms required by the lottery: (1) The full name, address, and license number of the person or entity shipping the machines; (2) The method of shipment and the name of the carrier, if any; (3) The full name, address, and license number of the person or entity to whom the machines are being sent and the destination of the machines, if different from the address; (4) The number of machines in the shipment; (5) The serial number of each machine; (6) The model number and description of each machine; and (7) The expected arrival date of the machines at their destination within the state.
