of video lottery machines out of the state. Any person shipping machines out of the state shall notify the lottery in writing prior to the shipment by providing the following information on forms required by the lottery: (1) The full name, address, and license number of the person or entity shipping the machines; (2) The method of shipment and the name of the carrier, if any; (3) The full name and address of the person or entity to whom the machines are being sent and the destination of the machines if it is different from the address; (4) The serial number of each machine; (5) The model number and description of the machine; (6) The lottery machine license number, if affixed; (7) The manufacturer of the machine; (8) The expected date and time of the shipment; and (9) The hard meter readings of the machine.
