for dependent coverage. An eligible employee may apply for dependent coverage within 30 days after the employee’s marriage or the birth or adoption of a child. A child may be covered from birth by notification of the commissioner by the employee. Payment shall be made by payroll deduction unless otherwise authorized by the commissioner.
