substances -- Annual report required. A person or entity required to report hazardous substances and remit the fee imposed by SDCL 34-12-21 shall file the report on the forms which include the following: (1) The name and address of the person or entity making the report; (2) The signature of a principal of the filer; (3) The status of the filer under SARA Title III; (4) The number of chemicals or mixtures and, in the case of filers reporting release information pursuant to section 313 of SARA Title III, quantity of hazardous substances handled or released; (5) The computation of the fee remitted; (6) The period covered by the report; and (7) Any other information that permits the department to determine whether the fee is properly computed and reported.
