67:10:02:04Incapacity -- Basis for payments for medical

examinations and reports -- Authorization required for intensive procedures. Payments for medical examinations and reports are made according to chapter 67:16:02. Routine X-ray and laboratory procedures considered necessary for the incapacity determination may be included. The individual must have prior authorization from the department if additional X rays or laboratory tests or an admission to a hospital is necessary to determine whether an incapacity exists.
