A notice shall contain the following information: (1) A statement of the action the department intends to take; (2) The reason for the intended action; (3) The specific regulation supporting the action; (4) An explanation of the individual's right to request a conference and to be represented by others at the conference; (5) The right to and procedure for requesting a fair hearing; (6) The right to be represented by others at the hearing; (7) The time limit for requesting a hearing; and (8) An explanation of the individual's rights.
