Based on recommendations made by the department's P and T committee, the department shall determine which drugs are subject to prior authorization. The provider must obtain approval from the department before supplying drugs subject to prior authorization. Drugs subject to prior authorization are listed on the department's website: http://www.hidsdmedicaid.com/.
