Article 67:15 (LIEAP), Article 67:16 (MS - Transportation) Article 67:18 (Child Support Enforcement), Article 67:42 (Family Foster Homes), Article 67:46 (Economic Assistance)Effective

The department shall develop a case service plan for each individual eligible for personal care services. The plan shall be based on the individual's needs assessment and shall contain items such as the following: (1) The reason for the service request; (2) The number of personal care service hours assigned to the individual; (3) The personal care services needed; and (4) The individual's responsibilities.
