requirements -- Provider. A facility is eligible for reimbursement for hospice services if the following conditions are met: (1) The facility has a signed provider agreement with the department; and (2) The facility meets the requirements of 42 C.F.R. §§ 418.52 to 418.116, inclusive, as amended to January 1, 2010.
