A service provided under this chapter must meet the following conditions: (1) The service must be medically necessary. Evidence establishing the medical necessity must be maintained in the individual's record; (2) The service must be outlined in the individual's care plan; (3) The service must be within the professional's scope of practice; (4) The service must be a direct, face-to-face, contact-care service with the individual; (5) The service may only be provided to individuals under age 21; and (6) The service must be provided by the school district in which the individual resides.
