in primary care provider. The primary care provider selection remains in effect until: (1) The recipient submits a written or verbal request for a provider change to the department during the recipient's annual redetermination of eligibility; or (2) The recipient or primary care provider submits a written or verbal request to the department. The department shall approve or disapprove the change request and shall notify the primary care provider and the recipient of the decision.
