Diabetes self-management training is limited to: (1) Ten hours of initial training; and (2) Two hours per year of follow-up training. The department shall authorize training that exceeds the limits set in this section, before the services are provided. Authorization must be based on documentation that is supplied to the department and which justifies the need for the additional services.
