The nursing facility shall provide the nurse consultant with the clinical record of each resident being reviewed. The resident's clinical record must include clear, concise, descriptive, and dated documentation that accurately described the resident's condition. The facility must make a resident's clinical record available to the department in a format acceptable to the department and must provide the department with copies of needed documentation from the resident's clinical record on request.
