portion of child care certificate -- Completion of request for payment form required for payment -- Provider to bill usual and customary charge. When the recipient has selected a qualifying provider, a copy of the child care certificate will be sent to the provider. To receive payment, the provider shall complete the request for payment within 60 days following the expiration date contained on the child care certificate. The unit price billed for the child care services provided must be the provider's usual and customary charge for the same services provided on the same date to the general public. When the recipient has changed providers, the department shall notify the previous provider in writing and shall indicate on the notice the revised expiration date.
