To participate in the delivery of TBI services, a provider must meet the following requirements: (1) Must be certified by the Department of Human Services as a community support provider. If the provider is located in another state, the provider must be licensed or certified by the applicable licensing agency from the other state either as an intermediate care facility for individuals with intellectual disabilities or as a home- or community-based service provider; (2) Must provide TBI services; (3) Must have a signed provider agreement with the Department of Human Services; and (4) Must have a signed Medicaid provider agreement with the Department of Social Services.
