Each agency shall establish a policy and procedures to collect and record client identification data, at the time of admission or as soon after admission as possible. Client identification data must be kept in the clinical file. Client identification data means: (1) Name, street address, and telephone number of the client; (2) Date of birth, gender, and race or ethnic origin of the client; (3) Unique identification number of the client; (4) Referral source; (5) Service start date; (6) Outcome measures; (7) Data for the state management information system; and (8) Any other client information as required by the department.
