67:62:14:06Renewal of eligibility -- Fee.

A qualified mental health professional (QMHP) shall register with the department for renewal of eligibility. A QMHP may register for renewal anytime during the year before the QMHP's current eligibility ends. A renewal registration must contain: (1) The QMHP's name and address; (2) The QMHP's current employer, or place of practice, with address and telephone number; (3) A copy of the QMHP's outh Dakota professional license or certificate; and (4) The renewal fee, as determined by the department.
