requiring notification. An appropriate regional facility shall notify the department and request a determination on continued designation if: (1) There is a change in the facility director; (2) There is a reduction in services provided by the facility; or (3) There is an impending closure of the facility. An appropriate regional facility shall provide the department with 30 calendar days written notice of closure.
