requirements. An eligible applicant shall submit a brownfields revolving loan subfund assistance application for an eligible site to the board on forms obtained from the department. The applicant may arrange with the department for a preapplication conference to provide for orderly application preparation. An application may not be considered by the board until the secretary has determined that the application is complete. A complete application must include the following items, as applicable to the proposal: (1) Application cover sheet; (2) Budget sheet; (3) Proposed method of financing; (4) Project narrative; (5) Cooperative agreements with units of local government; (6) Letters of financial commitment from other federal, state, or local agencies or private resources; (7) Status reports for all open and existing brownfields grants and loans; (8) Local government resolutions; (9) A financial statement, if requested by the department; (10) Business plan; (11) A department approved project remediation plan; and (12) A community involvement plan.
