The form for approval of continuing education programs must be completed by the sponsor and submitted to the board. The form must include the following information: (1) Name of the sponsor and address; (2) Name of the person in charge; (3) Location of the program; (4) Estimated number of the pharmacists and technicians participating; (5) General title of the program; (6) Type of program; (7) How the program objectives will be met; (8) Estimated contact time; (9) How attendance or participation will be proven; (10) How certificates will be awarded; and (11) A copy of any examination, if utilized.
