The pharmacist or intern transferring the prescription information shall: (1) Record on the original prescription: (a) The name and address of the pharmacy to which the prescription is transferred; (b) The name of the pharmacist or intern receiving the prescription information; (c) The name of the pharmacist or intern transferring the prescription information; and (d) The date of the transfer. (2) Record the number of refills transferred. If all refills are transferred, the original prescription shall be marked "void".
