The pharmacist or intern receiving the transferred prescription information shall: (1) Write the word "transfer" on the face of the transferred prescription; (2) Record on the transferred prescription: (a) The original date of issuance; (b) The original prescription number and the number of refills authorized on the original prescription; (c) The number of valid refills remaining and the date of the last refill; (d) The name and address of the pharmacy from which the prescription information is transferred; and (e) The name of the transferring and the receiving pharmacist or intern; and (3) Clarify verbally any unclear information on a facsimile.
